Appointments

Recommended Office Visits:Birth to two years: 1 or 2 weeks, 2 months,4 months,6 months, 9 months, 12 months, 15 months, 18 months. Two years and thereafter: Once a year Routine Physicals:Appointments for routine physicals will be made for patients within 8 weeks of contact with the physician's office. Routine Visits: Appointments for routine visits will be made for patients within 3 weeks of contact with physician's office. Persistent Symptoms: Patients with persistent symptoms must be seen within 48 hours of initial patient contact with the physician's office. Persistent symptoms are defined as symptoms with duration greater than 48 hours, of continuing persistence, that need medical care but are not urgent in nature. Immunizations:Birth to 1 year: 2 months, 4 months, 6 months, 12 months 1 to 5 years: 15 months, 18 months, 2 years, 4 years, 5 years Over 5 years: 11 years, 12 years
